CLINICAL, SUBCLINICAL FEATURES AND TREATMENT OF PATIENTS WITH SEVERE STRONGYLOIDES STERCORALIS INFECTION IN NATIONAL HOSPITAL FOR TROPICAL DISEASES AND BACHMAI HOSPITAL (2013 - 2019)

Abstract

Research on 30 severe strongyloidiasis patients, include hyperinfection and disseminated Strongyloid infection, who was diagnosed when strongyloid larvae were found in respiratory secretions, gastric fluid, or other sites outside the gastrointestinal tract at the Emergency Department, ICU of Bach Mai Hospital and the National Hospital for Tropical Diseases, we found that: The average age of patients in the study was 62.9 ± 13.4 years, 80 % had chronic diseases, of which 46.6% had a history of prolonged corticoid use. The most common diseases were pneumonia (46.7%), meningitis (13.3%). 36.7% of pa tients had sepsis and septic shock. Anemia was found in 76.7% patients, hypoalbuminemia: 89.2%, hypothyroidism: 42.8%. Only 16.7% of patients elevated peripheral eosinophils and 56.5% of patients had positive serology test for Strongyloidiasis. Overall, mortality rate in severe S. stercoralis infection up to 50%. Factors associated with a poor prognosis include shock, SOFA score at admission higher than 4 points, peripheral eosinophil is lower than 400 cells/mm3 and hypoalbuminemia is less than 20 g per liter.
