ASSESS THE RESULTS OF THE INTERVENTION MODEL INCREASE THE OPPORTUNITY OF INDICATIONS FOR TREATMENT WITH THROMBOLYTIC DRUGS IN PEOPLE WITH ISCHEMIC STROKE AT PHO NOI GENERAL HOSPITAL

Abstract

Pho Noi General Hospital has applied a multimodal intervention model in the pre-hospital, in-hospital, and hospital stages to improve the rate of patients with acute ischemic stroke receiving thrombolysis and the effectiveness of acute ischemic stroke treatment after leaving the hospital. We conducted this study to
evaluate the model’s results at Pho Noi General Hospital from January 2021 to June 2022 to determine the rate of patients arriving early, receiving thrombolysis, and the outcome. Treatment results for acute ischemic stroke compared to the period 2018 - 2020.
Results: The study recruited 318 cases of acute ischemic stroke. The rate of patients hospitalized earlier in all windows, in which the rate of hospitalization in the first 4.5-hour window (accounting for 29.6%) was higher than the previous period (19.6%). The average door-to-needle time in the post-intervention period was 43.8 minutes, statistically significantly lower than the pre-intervention period of 51.4 minutes. The proportion of patients with acute ischemic stroke who received thrombolysis achieved good treatment results, mRS 0-2, was 31.3%, not different from the pre-intervention period.
Conclusions: Our study shows that, after 1.5 years of applying a multimodal intervention model in managing stroke patients, it helps increase the rate of early hospital admission in all time windows, precisely a 4.5-hour window, helps shorten the door-to-needle time but has not improved treatment results.
