IMPACT OF ANTIMICROBIAL STEWARDSHIP PROGRAM ON USAGE OF LINEZOLID AT THE NATIONAL HOSPITAL FOR TROPICAL DISEASE

Abstract

Objectives: Analyze the effectiveness of clinical  pharmacological interventions based on linezolid usage
guidelines at the Central Tropical Diseases Hospital.
Subjects and methods: Interventional and longitudinal follow-up study was divided into two periods. The use of linezolid was evaluated based on a retrospective medical records of 245 patients before the intervention from July to December, 2021 and prospectively follow-up of 233 patients that having order form of linezolid with interventions by clinical pharmacists between April and September, 2022.
Result and conclusions: A total of 260 interventions were carried out with overall acceptance rate of 93.1%. In the intervention period, the appropriate rate of indications increased significantly from 72.7% to 99.5% (p < 0.001). Drug interactions with carbamazepine or metoclopramide were intervened to use alternative therapies. Serious adverse reactions, including hematological toxicities and lactic acidosis related to linezolid, are monitored, detected, and promptly stopped the relevant antibiotic. These results revealed the positive impact of interventions for the use of linezolid. Similar antimicrobial stewardship
program could be implemented for other highly-reserved antibiotics to enhance rational use of antibiotics in clinical practice
