A RETROSPECTIVE AND DESCRIPTIVE STUDY OF THE DISCLOSURE OF HIV STATUS TO CHILDEN AT CHILDREN'S HOSPITAL 2

Abstract

According to the UNAIDS Global AIDS Update, 37.9 million people were living with HIV in 2018, including 1.7 million children.  About 79% knew their HIV status (10). This rate in children is much lower.


A retrospective study at Children Hospital 2, Vietnam from 01/01/2007 to 31/03/2020 has been analyzed of HIV infected children more than 6 years old about epidemiological factor, clinic factors, viral load/CD4 in order to find risk factors and favorable factors of disclosure. The study also describe disclosure’s impact on adherence, progresst of treatment result, the child's psychology, their relationships with the family and the societ after the disclosure. 


Results: 536 HIV infected children more than 6 years old were followed up. There were 228 cases (42.54%) fully disclosed, 52 cases (9.7%) in the process of disclosure, 256 children (47.76%) not disclosed. The percentage in the group older than 12 years old were respectively 224 children (74.17%), 16 children (5.3%), 62 children (20.53%); and in the group 6-12 years old were 4 children (1.71%), 36 children (15.38%) and 194 children (82.91%).


The main reason for not- disclosed in the age group 6-12 years old is the caregiver’s afraid (84.45%); and in the group older than 12 years due to patient’s mental retardation (40.32%), caregiver's disagreement (24.19%).         


The study noted a better adherence as well as an improvement in Viral Load after disclosure (p < 0.05). Most children do not feel inferior or depressed after disclosure (270/273 cases, 98.9%).
