PREVALENCE OF HEPATITIS B INFECTION AMONG HEALTHCARE WORKERS AND ASSOCIATED FACTORS AT BACH MAI HOSPITAL IN 2019

Abstract

Objectives: This study was conducted to determine the prevalence of HBV among HCWs at Bach Mai Hospital in 2019, and to provide evidence for vaccination prevention interventions for HCWs in 2020.


Subject and methods: Quantitative cross-sectional study was conducted in November and December 2019 among 3,167 HCWs working at Bach Mai Hospital. Data on HBsAg and Anti-HBs were retrieved from the database of Department of Microbiology, Bach Mai Hospital. Logistic regression analysis was used to determine the factors associated with infection status and Anti-HBs among the participants.


Results and conclusions: A total of 3,167 cases were included in the analysis. The prevalence of hepatitis B was 5.56% (176/3167 cases). The rate of anti-HBs over 100 IU/mL was accounted for 50.93%. The remaining of 49.07% (1554 cases) need to be vaccinated or repeated vaccination. Among Internal medicine departments group, the Dermatology Department, Center for Tropical Diseases, and Traditional Medicine Department had the highest rates of hepatitis B infection (13.04%, 11.84% and 10.00% respectively). Other faculties were accounted for a lower rate, included Emergency Department (4.41%), Anesthesia - Resuscitation Department (3.52%), ICU Department (5.19%), Gastroenterology Department (5.06%) and Kidney Artificial Department (3.66%). In Surgery departments group, HCWs at Obstetrics - Gynecology Department had the highest rate of HBV with 13.33%. In Subclinical departments, Functional Exploration Department and Pathology Center had the highest proportion of HBV with 11.11% and 10.53%. HCWs were male and over 28 years of age were more likely to get hepatitis B than the others (OR = 0.57 and OR = 1.92, p <0.001). The age group of 18-28 years old also had lower Anti-HBs than that of the other age groups (OR = 0.62, p <0.001). The Administration/Function departments were more likely to get hepatitis B than the Internal medicine departments group (OR = 1.7, p = 0.01). Meanwhile, there was no significant difference between the Surgery departments, Subclinical departments, and Internal medicine departments. The overall rate of HBV infection among HCWs at Bach Mai Hospital in 2019 was 5.56%.  


Recommendations: There are 32.65% of HCWs need 3-doses and 14.62% need booster dose of HBV vaccine.
